Acknowledgment

By pressing the ACKNOWLEDGMENT button below I am agreeing to the following:
1) That the information I have provided on this form is true and complete to the best of my knowledge, and that I have not knowingly withheld or concealed any information;
2) That while SLR makes every effort to place only healthy dogs, SLR cannot guarantee the health of any dog and will not be held responsible for any medical expenses which may occur subsequent to the adoption;
3) That at the time of the adoption, I will be required to pay a nonrefundable Adoption Fee based on the age of the dog to help cover pre-adoption veterinary costs that include vaccinations, tests, spay/neuter, and other treatments;
4) That, if for any reason, the dog I adopt from SLR does not work out I am obligated to return it to SLR without exception and that I do not have the right to place the dog in any other home, shelter, or facility of any kind;
5) That will keep the dog I adopt from SLR up to date with all necessary vaccinations and tests, and will provide regular preventatives including heartworm, flea/tick, and any other medical needs as required
6) That I will ensure that the home living environment for this dog is generally safe, free of any potential escape paths, and free of the Poisonous Plants found on the reference list publicly available on the Schnauzer Love Rescue, Inc. website;
